Monday, January 4, 2021 San Juan Health Partners Family Medicine Farmington Open to Serve Patients without a Primary Care Provider

San Juan Regional Medical Center is opening up a new clinic in Farmington to serve our patients who do not have a primary care provider. San Juan Health Partners Family Medicine Farmington offers same or next day appointments upon discharge from the hospital, Emergency Department or Urgent Care for adult patients who have indicated they do not have a provider. The clinic is located across the street from San Juan Regional Medical Center at 656 West Maple Street. 

There is a great need for a clinic of this kind. We know that 30 to 40 percent of discharged patients from the inpatient or Emergency Department setting don’t have a primary care provider. Our goal is to meet the health disparities we see among our most vulnerable patients and avoid readmissions.

Caregivers at the clinic include one family medicine physician and two family nurse practitioners. Our hope is to provide a medical home for these patients to ensure they stay well.
