San Juan Regional Medical Center Goes Live with Information Technology Transformation
San Juan Regional Medical Center’s Information Technology transformation, Care Connect, has gone live. San Juan Regional Medical Center has been working with Cerner, a global leader in healthcare technology, for more than a year to prepare for the transition. The hospital implemented HealtheIntentSM, Cerner’s near real-time population health management platform designed to aggregate data from multiple sources and establish a comprehensive, unified view of an individual’s health and care experiences across the continuum of healthcare. This platform displaces more than 75 health IT applications.
Care Connect is changing the way San Juan Regional Medical Center works to provide safe and seamless health and wellness services to improve patient safety, accuracy, and efficiency. Benefits for patients include:
