San Juan Regional Medical Center congratulates Patrick Oglesby, DNP, FNP-C, a transitional care provider at San Juan Health Partners Family Medicine Farmington, for receiving the New Mexico Hospital Association’s Distinguished Service in Healthcare Award.

The award is presented by the New Mexico Hospital Association to select healthcare providers across New Mexico for their dedication to patient care and the joy they bring to their teams and the patients and families they serve. They make a difference in the lives they touch and represent the best in New Mexico healthcare. Oglesby was one of 14 caregivers statewide who received the award.

Oglesby was instrumental in implementing Family Medicine Farmington’s Transitional Care program, which started as a year-long pilot and has since transitioned into a permanent program. Oglesby works with patients who are discharged from San Juan Regional Medical Center’s Emergency Department and do not have a primary care provider to help them manage their health while working to find them the right primary care provider to meet their specific needs. 

“We are so grateful for the Creative Vitality, Sacred Trust and Personal Reverence that Patrick displayed designing this program to help our patients with one of the biggest needs we see year after year on our Community Health Needs Assessment- Access to Care,” said Jason Rounds, President and CEO of San Juan Regional Medical Center. 

The program has seen tremendous success. In fact, one patient who worked with Oglesby after being hospitalized with a blood glucose level of over 1,600 was able to get his A1C down to an 8.2 from a 16.6. 

“It’s not a stretch to say that Patrick Oglesby put me on a path that will save my life. He is dedicated to a healthy recovery and instilling the knowledge and resources for general wellness. The web of referrals that he provides creates a complete network of care,” the patient said. “When he makes that final referral to a new PCP, it is a jolting experience. You’ve gone from crawling to walking and he sets you up to be able to run with your PCP.”

Oglesby holds a Doctor of Nursing Practice – Family Nurse Practitioner from Colorado Mesa University in Grand Junction. He earned both his Bachelor of Science in Nursing and Associate of Science in Nursing from Excelsior College in Albany, New York. He has been a caregiver at San Juan Regional Medical Center for more than 25 years, with 23 as a Flight Nurse and Paramedic with AirCare.
